Tax Forms for Individuals

 


The Main Library and all branches have basic tax forms available during tax season. 

 

AARP Tax Aide Volunteers offer free tax advice for senior citizens and low to moderate income families.  AARP membership is not required.  Among other locations throughout Stark County, AARP Tax Aide is offered at the Main Library on Fridays from 9 a.m.-2 p.m.; Lake Community Branch Library on Mondays and Fridays from 9 a.m-2 p.m.; and North Branch Library on Wednesdays from 9 a.m.-2 p.m. through April 15.

Federal Income Tax Forms - IRS  Find forms, new tax law changes, get a copy of your return, E-File your taxes, find out about your refund and much more.  For questions about federal taxes you can call the Internal Revenue Service at 1-(800) 829-1040 or contact the local office at (330) 588-4417. Federal tax forms can also be requested by calling the IRS Forms Hotline at 1-(800) 829-3676.

State of Ohio tax forms can also be requested by calling 1-(800) 282-1782.  For refund status information call the OhioIncome Tax Refund Hotline at 1-(800) -282-1784.

Ohio Department of Taxation - School District Income Tax
In Ohio, school districts are permitted to levy an income tax subject to voter approval. As of January 2014, income taxes were in place in 188 school districts.
